Three vaccine clinics remain before Thanksgiving break.
On-campus Pfizer clinics before Thanksgiving break are 11/9, 11/17 and 11/19 for first, second or booster doses.
Three on-campus Pfizer vaccine clinics remain before the University closes for the Thanksgiving break at the end of the day on Friday, Nov. 19. You may sign up for one of the clinics below, or go to the University Health Center on weekdays, 8 a.m.-4:30 p.m. to receive a vaccine.
Below are upcoming Pfizer vaccine clinics at Johnson Commons. More information is on UM’s vaccination webpage.
Tuesday, Nov. 9, 1-3 p.m.: Make 11/9 Pfizer appointment.
Wednesday, Nov. 17, 9 a.m.-12 p.m.: Make 11/17 Pfizer appointment.
Friday, Nov. 19, 9 a.m.-12 p.m.: Make 11/19 Pfizer appointment.
Please print these two medical paperwork forms below to fill out and bring with you to your on-campus vaccination. Blank paperwork will also be available at the vaccination site. If you are getting a second or booster dose, remember to bring your CDC vaccination card with you and fill out and bring the below self-attestation form.

Vaccines are also available at all county health department clinics. Appointments can be made through https://covidvaccine.umc.edu or by calling the COVID-19 hotline at 877-978-6453. Off-campus vaccination sites are on this webpage.
All University employees (including faculty, staff, graduate and undergraduate student employees) should receive both shots of a two-dose vaccine, or one shot of a one-dose vaccine, by Jan. 4. More info from the Chancellor is here. More information about the employee vaccine mandate is here.
